HIPAA 

Your individually identifiable information specifically related to information about your healthcare, for example, dates you receive healthcare services, your payment for health services, etc., is Protected Health Information or “PHI”. If we solicit PHI from you through our website outside of scheduling services, processing payment for our services and internal operations (e.g., through an email that links you to your patient portal where you may complete a survey), we will inform you of the purpose for collecting the PHI and you will have the option to decline to provide us with your PHI for the specified purpose.

Collection of Anonymous Information

We use technology to passively collect and store certain anonymous information that is not tied to a specific identified user which may include, but is not limited to your Internet protocol address, browser type and operating system. 7.5. Assignment.

We shall have the right to assign and/or transfer these Terms and our rights and obligations hereunder to any Third Party after notifying you as provided for herein. You agree and acknowledge that you shall not assign or transfer your rights or sub-contract or delegate the performance of any of your obligations under these Terms without our prior written consent in our sole and exclusive discretion.

7.6. Rights of Third Parties.

7.10 Conflicts with Other Agreements
